TL;DR

Provide non-clinical navigation support to Medicare patients.

  • Serve as the primary point of contact for patients after their intake visit.

  • Coordinate with clinical team members involved in a patient's care, while providing non-clinical support only.
  • Requirements Certified Nursing Assistant (CNA) certification / state registry in good standing; or equivalent hands-on patient-care and patient-navigation experience. 2+ years of patient
  • or community-facing support experience (care coordination, case management, member advocacy, health coaching, benefits navigation, community health work, or similar).
  • Working knowledge of the U.S. healthcare system and Medicare.
  • Comfort working independently, managing a caseload, and maintaining strong documentation.
  • Reliable computer, internet, and a quiet space for patient calls.
