TL;DR
Provide non-clinical navigation support to Medicare patients.

- •Coordinate with clinical team members involved in a patient's care, while providing non-clinical support only.
- •Requirements Certified Nursing Assistant (CNA) certification / state registry in good standing; or equivalent hands-on patient-care and patient-navigation experience. 2+ years of patient
- •or community-facing support experience (care coordination, case management, member advocacy, health coaching, benefits navigation, community health work, or similar).
- •Working knowledge of the U.S. healthcare system and Medicare.
- •Comfort working independently, managing a caseload, and maintaining strong documentation.
- •Reliable computer, internet, and a quiet space for patient calls.
