About the role
TL;DR
Provide non-clinical navigation support to Medicare patients.
- •Serve as the primary point of contact for patients after their intake visit, helping them understand and act on their care plan.
- •Key Responsibilities Help patients understand and act on their care plan.
- •Resolve navigation friction: portals, referrals, appointment logistics, forms, and provider communications.
- •Identify practical and social-needs support and connect patients to available resources.
- •Coordinate with clinical team members involved in a patient's care.
- •Document every patient interaction and support activity.
- •Requirements Community Health Worker (CHW) certification or equivalent experience. 2+ years of patient
- •or community-facing support experience.
- •Working knowledge of the U.S. healthcare system and Medicare.
- •Comfort working independently, managing a caseload, and maintaining strong documentation.
- •Reliable computer, internet, and a quiet space for patient calls.
