About the role
TL;DR
Connects patients with healthcare resources and builds community trust.
- •As an Outreach Specialist/Care Coordinator, you will be Waymark's frontline presence, connecting patients with healthcare resources and building trust within communities.
- •You will be part of a multidisciplinary care team.
- •Key Responsibilities Initiate outreach to new patients and connect them with healthcare resources.
- •Foster trust and improve patient engagement through empathetic communication.
- •Collect and analyze patient data to monitor care and improve preventative care utilization.
- •Collaborate with primary care providers and the Waymark care team.
- •Document patient encounters accurately and contribute to workflow refinement.
- •Requirements Excellent verbal and written communication skills.
- •Comfort tailoring communication to physicians and patients.
- •Highly organized and self-motivated to work independently.
- •Customer service and/or sales experience.
- •Experience with Electronic Health Records (EHR).
- •Experience in a healthcare delivery setting.
- •Passion for working with diverse communities.
- •Reside in Greater Hampton Roads with access to a car.
