Manage and clinically oversee a remote team of ECM staff, ensuring high-quality care for clients.
- •VyncaCare is seeking an ECM Clinical Manager to provide remote clinical oversight and management of Enhanced Care Management (ECM) staff.
- •This role focuses on addressing client needs, including medical, behavioral health, and social determinants of health, while collaborating with care teams and monitoring staff performance.
- •Key Responsibilities Supervise a team of 10-15 ECM staff to ensure high-quality care management services.
- •Oversee client outreach, enrollments, and the creation of Individual Care Plans.
- •Drive the team to meet key performance indicators (KPIs).
- •Provide clinical oversight, including supervision, clinical sign-offs, and leading IDT meetings.
- •Collaborate with the Director(s) of Enhanced Care Management on process improvements.
- •Requirements Active, unrestricted LCSW or LMFT license in California. 2 years of people management experience (4 years preferred). 2-3 years of care management experience. 1-2 years of post-acute experience.
- •Proficient in Microsoft and Google applications; Salesforce experience is a plus.
