- •Pair Team is seeking a Case Management Specialist to join their mission-driven team, reimagining how Medicaid and Medicare serve underserved populations.
- •This role focuses on delivering whole-person care by partnering with community organizations.
- •Key Responsibilities Maintain ongoing caseload through evidence-based approaches to promote engagement and health goal achievement.
- •Utilize relationship-based strategies to support members with social support navigation.
- •Conduct periodic telephonic and SMS outreach to ensure timely follow-up.
- •Collaborate with a multidisciplinary team to support members with complex needs.
- •Requirements 1+ years of general work experience (Case Management preferred).
- •Physically located in California.
- •Strong understanding of cultural fluency and high degree of empathy.
- •Ability to work collaboratively in a multidisciplinary team.
- •Strong technical skills and comfort with technology innovation.
