Bilingual RN Care Manager to support patient care plans remotely.
- •Pair Team is seeking a Registered Nurse Care Manager to provide whole-person care to underserved populations.
- •This role involves supporting patient-driven care plans to improve outcomes for individuals with complex medical needs, Serious Mental Illness, Substance Use Disorder, or homelessness.
- •Key Responsibilities Support a caseload of individuals with complex medical needs.
- •Develop and implement Shared Care Plans with individuals.
- •Educate individuals on medical and behavioral health conditions.
- •Collaborate with a multidisciplinary care team to address barriers to care.
- •Build trust and relationships with individuals facing complex challenges.
- •Requirements Active Registered Nurse license issued by the state of California. 2-3+ years of experience working for a health plan or at-risk provider.
- •Bilingual in English and Spanish.
- •Strong technical skills and comfort with new technology.
- •Quiet and HIPAA-compliant at-home work environment.
