- •The Nurse Practitioner (NP) is the primary care provider, delivering comprehensive family care in the home setting.
- •You will collaborate with an interdisciplinary team to improve whole-family health.
- •Key Responsibilities Manage a panel of families as the primary care provider, offering longitudinal primary and preventive care.
- •Utilize Nest Health’s electronic health record and care management tools.
- •Propose treatments for chronic and infectious diseases and prescribe appropriate medications.
- •Provide information and assistance to families on health and social care services.
- •Requirements Active, unrestricted license to practice in Arizona.
- •At least 2 years of experience providing clinical services to pediatric and adult patients.
- •Proficiency in clinical nursing skills such as blood drawing and patient assessments.
- •Available to work a full-time, five-day work week and participate in a rotating call schedule.
