TL;DR
Provide in-home health risk assessments for patients across New Jersey.
- •Porter is expanding into New Jersey and is looking for Nurse Practitioners to provide in-home health risk assessments.
- •This is a state-wide traveling position supporting patients across New Jersey.
- •Key Responsibilities Conduct in-home health risk assessments.
- •Evaluate past medical history, review of systems, and medication reconciliation.
- •Perform vital signs and comprehensive physical exams.
- •Identify diagnoses for care management and active medical management.
- •Formulate a list of current and past medical conditions.
- •Requirements Valid unencumbered New Jersey NP license. 3 years of patient care experience required.
- •Experience in primary care/adult/geriatric, and EMR skills preferred.
- •Strong sense of empathy, compassion, and friendliness.
- •Exceptional communication skills, both written and oral.
