About the role
TL;DR
Provide on-site support for Medicare 855B inspections and manage Chronic Care Management.
- •Serve as the on-site point of contact for Medicare 855B site inspection and support Chronic Care Management (CCM) program.
- •Key Responsibilities Act as the immediate point of contact for MAC field inspectors.
- •Escort inspectors through the facility and respond to operational questions.
- •Facilitate patient enrollment into Flagler's CCM portal.
- •Make daily outbound calls and send text messages to patients.
- •Collect and document patient health information.
- •Requirements Active LPN or LVN license in WI.
- •Minimum 3 years of experience as an LPN or LVN.
- •Proficiency with Zoom and Slack.
- •Personal laptop with a working microphone, camera, and reliable internet connection.
- •Calm, articulate, and professional conduct.
