About the role
TL;DR
Clinical Lead Care Manager coordinates client care, manages cases, and facilitates access to services.
- •Join Vynca as a Clinical Lead Care Manager (CLCM) to provide comprehensive care for individuals with complex needs.
- •You will be the primary point of contact for clients, coordinating with their providers to ensure aligned care and facilitate access to services.
- •Key Responsibilities Assess member needs in physical health, mental health, SUD, oral health, palliative care, memory care, trauma-informed care, and social supports.
- •Oversee the development of client care plans and goal settings.
- •Offer services in person, via telehealth, or in the field, connecting clients to social services and advocating on their behalf.
- •Utilize evidence-based practices and maintain patient health records in the EMR system.
- •Conduct outreach and engagement activities and complete monthly reporting for program compliance.
- •Requirements LPCC, LCSW, or LMFT license in California required. 1+ year of experience as a care manager, care navigator, or community health worker supporting vulnerable populations.
- •Willing and able to work Monday-Friday 8:30am-5:00pm, both in the field and remotely.
- •Working knowledge of government and community resources related to social determinants of health.
- •Valid driver's license and reliable transportation.
