- •Join Vynca as a Clinical Lead Care Manager to provide comprehensive care for individuals with complex needs, ensuring coordinated and aligned healthcare services.
- •You will be the primary point of contact for clients, partnering with healthcare providers to facilitate access to care and promote member empowerment.
- •Key Responsibilities Assess member needs across physical health, mental health, SUD, and social supports.
- •Develop and oversee client care plans and goal setting.
- •Connect clients to social services and advocate on their behalf with healthcare professionals.
- •Utilize evidence-based practices like Motivational Interviewing and Trauma-Informed Care.
- •Conduct outreach, engagement, and maintain client records in CRM and EMR systems.
- •Requirements Active California Licensed Vocational Nurse (LVN) license. 2+ years of experience as a care manager, care navigator, or community health worker.
- •Willingness to travel up to 5 days per week within the San Diego area.
- •Valid driver's license and reliable transportation.
- •Working knowledge of government and community resources related to social determinants of health.
