About the role
TL;DR
Coordinate patient care and therapy initiation for Praxis therapies.
- •Serve as the primary coordinator for patients, caregivers, and providers to support initiation and maintenance of Praxis therapies across the patient journey.
- •Key Responsibilities Manage complex patient caseloads and oversee case coordination from onboarding through adherence.
- •Identify and resolve access and adherence barriers to expedite time to therapy.
- •Provide individualized support and follow-through for patients and caregivers.
- •Collaborate with healthcare providers and internal teams to ensure timely task completion.
- •Requirements Experience in patient case management, preferably for rare or refractory CNS conditions.
- •Strong communication and coordination skills to manage patient journeys.
- •Ability to travel as needed and work remotely.
- •Organizational skills to manage timely completion of tasks.
