About the role
TL;DR
Provide non-clinical navigation support to Medicare patients.
- •As a Care Advocate, you'll support patients in navigating their healthcare journey, helping them understand their care plans and connect to necessary resources.
- •Key Responsibilities Serve as the primary point of contact for patients after their intake visit.
- •Help patients understand and act on their care plan, including scheduling appointments and using their Medicare benefits.
- •Resolve navigation friction by managing referrals, appointment logistics, and provider communications.
- •Identify and connect patients to practical and social-needs support.
- •Coordinate with clinical team members while providing non-clinical support.
- •Document every patient interaction and support activity accurately.
- •Maintain confidentiality and comply with HIPAA and privacy standards.
- •Requirements Active, unrestricted LPN/LVN license. 2+ years of patient
- •or community-facing support experience.
- •Working knowledge of the U.S. healthcare system and Medicare.
- •Comfort working independently and managing a caseload.
- •Reliable computer, internet, and a quiet space for patient calls.
