About the role
TL;DR
Deliver high-quality telehealth care to older adults.
- •Bold is seeking NPs/PAs with a strong background in preventive and lifestyle medicine to join our clinical team.
- •You’ll be at the forefront of delivering high-quality relationship-based care to older adults across the US.
- •Key Responsibilities Deliver high-quality telehealth care focused on weight management, prevention, chronic disease management, and lifestyle interventions (e.g. nutrition, movement, sleep, stress) to older adults (65+).
- •Provide feedback on the development of clinical protocols, documentation standards, and scalable workflows.
- •Use EMR for documenting patient information and communicating to other health care providers.
- •Partner with care coordinators, health coaches, and other clinicians to develop personalized care plans.
- •Requirements Active and unrestricted NP/PA licenses in multi-states including CA, TX, and/or PA. 5+ years of clinical experience. 2+ years experience in telehealth with high proficiency with EMRs & communication tools.
- •Strong foundation in preventive and lifestyle-focused care including management of weight, hypertension, diabetes.
- •Experience with GLP-1s.Excellent communication skills and empathy-driven clinical approach.
