• •They are seeking a Clinical Pharmacist Lead to build and lead their medication management function, focusing on population health programs.
• •Key Responsibilities Build and lead pharmacist-led medication management workflows.
• •Personally manage a patient panel to establish clinical and operational standards.
• •Design protocols for medication initiation, titration, monitoring, and adherence.
• •Hire, onboard, and develop clinical pharmacists and support staff.
• •Drive performance through data and insights to improve clinical outcomes.
• •Requirements PharmD required; active pharmacist license in DC, MD, VA, GA, TX. 5-8+ years of clinical pharmacy experience, ideally in cardiology, ambulatory care, or population health.
• •Experience with heart failure management and chronic disease management.
• •Demonstrated ability to move from individual contributor to people leader.
