VP/Director of Care Operations & Transformation

Lead care operations and transformation, championing AI-enabled delivery.

• •We're looking for a VP of Care Operations & Transformation to build and lead Chamber's care model.
• •You'll design workflows, implement technology, and drive performance, championing AI-enabled care delivery.
• •Key Responsibilities Recruit, train, and scale care teams that proactively engage patients.
• •Lead the adoption of AI-driven outreach, documentation, quality monitoring, and decision support.
• •Own day-to-day care operations: performance dashboards, staffing models, QA programs, and capacity planning.
• •Requirements 12-15+ years in healthcare delivery operations, preference for value-based or virtual care experience.
• •Experience in a scaling start-up environment; comfortable rolling up your sleeves and building and operating through ambiguity.
• •Demonstrated success as tech-forward operator, excited to deploy automation and AI into care delivery.
