About the role
Enroll patients referred to Pomelo Care by health plans, providers, and hospital systems.
- •The Referral Specialist is responsible for enrolling patients referred to Pomelo Care by health plans, providers, and hospital systems.
- •You will educate patients on clinical and social resources while driving enrollment through multi-channel outreach.
- •Key Responsibilities Conduct outbound calls, SMS, and email outreach to drive new patient enrollments and meet productivity goals.
- •Document all incoming referrals and interactions accurately in systems of record with timely updates.
- •Coordinate with health plan case managers and external providers to share referral updates and achieve enrollment targets.
- •Requirements Bachelor's degree in social work, healthcare, or a related field.
- •Excellent customer service and communication skills with experience in healthcare referral systems.
- •Familiarity with Medicaid health plans and the ability to work independently in a remote environment.
